RELEASE FROM LIABILITY.  I fully and forever release, and discharge Youth Education Through Sports, Inc., the  New York City Department of Parks & Recreation, the City of New York, the State of New York, Madison Square Boys & Girls Club Pinkerton Clubhouse and their subsidiaries, directors, officers, employees, agents, insurers, sponsors, advertisers, owners or operators of the Event, facilities, equipment, and vehicles, and all others involved in the Event (the “Released Parties”) from any and all injuries (including death), losses, damages, claims (including negligence claims),demands, lawsuits, expenses, and any other liability of any kind, of or to me, my property, or any other person, directly arising out of or in connection with my participation in, or attendance at the Event, including transportation related to the Event, even if it is due to the negligence or other fault of the Released Parties.

I hereby irrevocably consent and authorize YOUTH EDUCATION THROUGH SPORTS, Inc. and its employees, staff, and agents to use my likeness, voice recordings and/or photograph (still or motion picture of videotape) or any other representation of me in any form, including but not limited to magnetic, audio or video/digital recording, in any manner or method of legitimate exhibition, presentation, transmission or reception, in whole or in part.

I hereby release and discharge YOUTH EDUCATION THROUGH SPORTS, Inc. and its employees, staff, and agents from any and all liability arising out of the use, distribution and presentation of photographs and recordings and other representations authorized under this consent.

I hereby confirm that the participant enrolled below has no existing medical, physical, psychological, or emotional condition which prohibits him/her from participating in the activities included in the SMARTBALL Initiative.
